Student Handbook and Information

I authorize Region Five Adult Education to photograph me. I give my consent to use photographs of me to Region Five to publish in public materials produced by Region Five for the express purpose of outreach or recruitment or any other lawful use in conjunction with Region Five activities. I understand and agree that I will not receive payment or royalties as a result of the use or publication of any photographs, nor will I be entitled to notification of use or publication. I have voluntarily agreed to give my authorization and consent.
